Chronic fatigue assessment social story

I have been referred to the chronic fatigue syndrome ME service. The service helps with post-viral fatigue.

My parents or carers will have got a letter and will know where to go.

My assessment will be at Norfolk Park Medical Centre.

We will go to the main reception and let them know we are here.

They will ask me for my name, who I am seeing, and what time my appointment is.

My parent or carer will know this.

We will then go up the lift or stairs to the top floor.

We will wait in the waiting area with red chairs for my appointment.

A member of the team will come to find us when they are ready. They will walk with me and my parent or carer to the room.

This is where my assessment will take place.

The room may look like this. Or it might look a different way too.

We will meet a doctor.

I might meet an occupational therapist.

In the assessment, we will be asked some questions. My parent or carer is allowed to help me.

  • symptoms
  • school and home life
  • mood
  • activity
  • sleep
  • family history

We can also ask questions too.

Then we will wait in the waiting area again while they talk about a care plan for me.

After, they will ask us to come back into the room to talk ask some more questions.

They will talk to us about my care plan and make sure we are happy with it.

We can also ask more questions.

Then the appointment is finished.

My parent or carer will take me back home or to school.
