Now you are a teenager it is time to start thinking about who will look after you at the adult hospital. It is important that we have time to plan this and for you to meet the people who will be looking after you in the future, so we do this in stages. You will be seen in the “transition clinic” until you are ready to be transferred to the adult team.
In the transition clinic there will be your Sheffield Children’s Urology Consultant, Miss Rusu or Miss MacDonald, and an adult Urology Consultant from Sheffield Teaching Hospitals.
There will also be Louise, a Sheffield Children’s Urology Nurse, and 2 adult Specialist Urology Nurses, either Rachel, Cecilia or Marie.
At times there may also be other professionals present who may be helpful to you when planning your transition to adult services.
Until you have your first clinic appointment in adults, you will remain under the care of the Children’s Hospital. You should still phone the team at the Children’s Hospital with any questions or to request supplies as you have always done.
The transition clinics are at the main outpatient’s department at Sheffield Children’s Hospital. We do these face-to-face so you can meet the adult teams.
As well as reviewing your current urology care, we will also explain the process of transitioning to adult services, and will discuss with you where your adult urology care is best transferred.
If your care is likely to stay in Sheffield, then we will introduce you to the new doctors and specialist nurses in the adult team. Please feel free to ask any questions you may have, such as your medical problems, jobs and careers, or personal relationships, and so on.
We will discuss this with you and then make a shared decision about where your care is best transferred to. This may be to a hospital nearer home or to stay in Sheffield.
At some point between 16 and 18 years your care will transfer. We will listen to you and between us decide when is a good time for your care to move, you may decide you are ready at 16 year or you may wish to stay nearer to your 18th birthday. It is a good idea to move when other things in your life are calmer, such as not in the middle of exams.
