Moving from Sheffield Childrens Hospital to an adult service.
Your doctors and nurses will start to talk to you about transition at around 14 years old.
You are getting older and need to be cared for by adult teams.
You might be given more responsibility and independence.
If you live in Sheffield your care will stay in Sheffield.
If you live out of Sheffield, your care may be within your local service.
If you are still coming to SCH you can come to the Emergency Department here.
If you have moved to adult services, you will go to the adult hospital Emergency Department.
If you are over 16 years old and access the mental health services, you will need to attend the adult hospital Emergency Department.
Your parents, cares, doctors, nurses or anyone who looks after you can still help you.

A communication and health passport may be used to help you share your needs
We understand this may make you feel worried, talk to the people that look after you about your feelings.
This is a group of young people who meet to talk about how the hospital can be made better.
