Rheumatology therapy transition

What is transition?

Transition is the process of moving your care from children’s to adult services. The purpose of transition is to:

  • treat you more independently and more like an adult
  • let you ask questions
  • help you to understand your condition and how you can manage it
  • prepare you to go to adult therapy services

You will discuss transition in your clinic and therapy appointments. This is some information about your therapy transition.

The adult therapy team

The young person adult therapy team has both physiotherapists and occupational therapists.

Your first appointment may be a joint appointment with both the physiotherapist and the occupational therapist, or you may see them separately.

Where are the team based?

The young person adult therapy team are based at the Royal Hallamshire Hospital in Outpatients.

How do I get referred?

Your therapists at the Sheffield Children’s Hospital will discuss with you your referral onto the young person’s adult therapy services. They will speak to the adult team to organise this. If you need therapy in the future, you can speak to your consultant and get referred.
