Welcome to the Pain Management Team.
We are keen to help and have already put your child’s name on our waiting list.
We aim to offer you an appointment as soon as we can, however, there is a high demand to see our Pain Team specialists, and we have limited slots per clinic.
You should have received a text message with a link to a set of pain questionnaires. Please make sure you complete these before your appointment.
There is a part for your child, and a part for you. If your child is not able to fill in the questionnaire themselves, please fill in the parent and carer section only.
This provides us with essential information about your child that will support your appointment. It will also help you to think more clearly about your child’s pain and what you would like to tell us about it when you come to see us.
Do not worry if you find some of the questions do not seem to apply to you or if you are not sure how to answer. You will be able to talk about anything that you are unsure of when you visit us.
We will re-send the questionnaire when your child is near the top of the waiting list.
You will receive another questionnaire 6 months after your appointment, another one 12 months after, and a final one 24 months after your first appointment.
This is so we can keep track of how you are doing.
You will receive a text message or letter to arrange an initial appointment for the Pain Clinic when your child’s name is near the top of the waiting list.
We have a cancellation list if you are able to accept an appointment at short notice (usually 24 hours). If you would like your child’s name adding to this list, please call 0114 226 0843.
New patient assessment appointments are 90 minutes long.
