The professionals in the Eye Care Team (orthoptists, nurses, optometrist and ophthalmologists) need to be able to share information about you and your child with other professionals and members of the health care team (such as teachers including those from the service for children with a sensory impairment).
Examples of the information we may need to share are: details of your child’s eye condition, vision and appointment details.
In order to give the best possible care, we want to share information about your child with others who can support your child further.
The information provided by the eye care team may be used to provide advice and support to other professionals so your child gets the maximum benefit.
The information will be shared with authorised people in a secure manner. This may be verbally, by written reports or electronically.
Other professionals working with your child will receive timely and accurate information so that your child’s individual needs can be met appropriately.
Yes, you and your child’s information with be treated with confidentiality. For more information about how we use your information, please visit our resource.
You can withdraw your permission at any time. Please inform a member of the eye care team.
If you do not give permission, the treatment and care of your child at Sheffield Children’s by the eye care team will not be affected in any way.
